If you would like for Dr. Holt to come to your home to assess your pet, we now offer in-home quality of life assessments 

Cost of a Quality of Life Assessment and what's included:
  • $250
  • This includes the at-home visit, a full physical exam, consultation, recommendations and treatment options (if available/necessary)
  • Dr. Holt will not be able to run any diagnostics (bloodwork/x-rays)
  • Medications can be prescribed in certain instances but not for all patients
  • The cost of the assessment is for a 45 minute appointment
  • If we decide that euthanasia is the best option the day of the Quality of Life Assessment, we will deduct $100 from the total bill.
  • If you decide to use our services for euthanasia at a later date then $50 will be removed from your bill at the time of euthanasia. 
  • Once you contact Dr. Holt and make the appointment, she will email you a Questionnaire and a Quality of Life Scale that will need to be filled out prior to your appointment in order to help Dr. Holt give you the best advice possible.
  • Dr. Holt's full assessment and recommendations will be emailed to you within 24 hours of the appointment.
